Our History

ChapCare has been putting patients first since 1998. For 22 years ChapCare has served the communities of the San Gabriel Valley, providing quality health care for men, women and children of all ages. Since 2013, ChapCare has been recognized as a Patient Centered Medical Home (PCMH). A PCMH is a team-based model of care led by a physician who provides continuous and coordinated care throughout a patient’s lifetime to maximize health outcomes.

Who We Serve

ChapCare serves all residents of the San Gabriel Valley. We contract with a number of qualified health plans to provide primary healthcare services for those with health insurance. ChapCare also serves as the community safety net by providing comprehensive healthcare to low income and uninsured individuals and families.
